If you are a new customer with Eastside Utility or an existing customer requesting service at a different address, you can apply in the following ways.
Online
- Complete the Water Service Contract on this page and save it to your computer. Email the completed contract to contract@eastsideutility.com.
- You may also email questions to contract@eastsideutility.com.
- Attach a digital copy of the signer’s driver’s license to your email. You may send the contract and license in separate emails. Pay the $50.00 service fee using the link below.
By fax
Fax the contract and a copy of the driver’s license to 423-490-9516. If you fax the contract, call the office at 423-892-2890 to pay the $50.00 service fee by credit card.

Tips for using online fillable forms
